Where the click dies
This one forks. Acuity is usually set up one of two ways, and each cuts the line in a different place.
Where it dies
The click dies twice, once per arrangement. Inside an embed the appointment is booked and the parent page sees nothing of it unless the platform emits something the page can listen for. On a separate scheduling domain the appointment is also booked, and campaign identity has to survive the hop between hosts; where nothing was set up to carry it, the scheduler takes the credit.

The audit establishes which of these is live on the venue, and which is worth fixing first.

Why nobody catches it
Routine reports compare totals, not the path through the embed or scheduling domain. Only a controlled appointment exposes that boundary. A stable booking total does not clear the tracking.
